SRVS is seeking a Community Life Manager.
Under the direct supervision of the Residential Director, the Community Life Manager (CLM) is responsible for providing overall comprehensive care to assure that the people being supported thrive through coordination of medical, social, financial, religious, recreational, work, housing, employment or day services. The CLM will assure that staff supports are in place for the person supported to live as full participants in their home and community utilizing a person centered plan to develop, implement and monitor the Individual Support Plan (ISP) while focusing on the environment, health, safety and well being of the person supported.
